A non-profit multi-specialty organization in Brooklyn, NY, is seeking a Nursing and Community Care Manager to join their team. Their mission is to improve the health of underserved communities, one block at a time. In close collaboration with community-based organizations, local providers, and leading health plans, we are reorganizing the health system to focus on what matters to our members. Equipped with world-class, custom care delivery technology, they deliver personalized primary care, behavioral health, and social services to deliver a radically better experience of care for every member and community they serve.
Compensation and Benefits for the Nursing and Community Care Manager:
The starting salary range for this position is $97,000 – $119,000 | yr
This is a Full-time position

Requirements of the Nursing and Community Care Manager:
Must have an active, unrestricted NYS license
Must have at least 5+ years of experience providing clinical services to Adult and Geriatric individuals with co-occurring chronic medical and behavioral health conditions.
Must have 3+ years directly managing a team of RNs (or higher clinical training)
Must be willing to travel within the community (home-based member visits) and its healthcare systems (hospitals and rehab centers)
Bilingual Spanish Preferred
Responsibilities of the Nursing and Community Care Manager:
As the Manager, Nursing and Community Care, you will provide clinical, administrative, and managerial support to the RN team Hub Registered Nurses and Care-team Registered Nurses
You will also partner with the Care Team Lead and other market leaders to support our interdisciplinary care teams
