Primary responsibilities:

Responsible for providing accurate, complete and appropriate coding using ICD-9-CM, ICD-10-CM/PCS and CPT classification systems. Assigns appropriate APC grouping according to diagnoses specified in the record by the treating provider(s) and in keeping with regulatory requirements. Abstracts relevant clinical and demographic information from the medical record.

Qualifications:

Certification as Certified Clinical Specialist (CCS) required. Working knowledge of the ICD-9-CM and CPT coding systems, medical terminology, anatomy and physiology. Experience in computer operations. A minimum of two (2) years hospital coding experience or equivalent, preferred.Successful completion of Reid Hospital’s orientation and competency based skills appropriate for the job. Individual performance and competency based skills are maintained through ongoing assessment of competence and educational activities. Will receive written evaluations on an annual basis.

Additional info:

Codes complex outpatient medical records for the purpose of quality planning and assessment, reimbursement, research and compliance with Federal regulations according to ICD-9-CM, ICD-10-CM/PCS and CPT-4 classification systems. Performs data entry/abstraction on all patients into the applicable computer system. Calculates the proper APC assignment. Assists providers with appropriate diagnosis determination as needed. Keeps current with coding guidelines, rules and regulations and new codes. Refers coding and system questions to the Coding Manager/Director in a timely manner. Assists with abstraction and retrieval of data for special studies. Performs medical record documentation review for reporting to the Medical Records Committee. Assists other departments with coding functions as needed. Assists Clinical Documentation Specialists with coding functions as needed. Follows compliance with all laws, regulations, and guidelines of federal and state programs with an emphasis on prevention of fraud, waste and abuse. Performs special assignments as required.
