(Physician/MD qualifications required) Palliative Medicine – MASSACHUSETTS, Boston Seeking a Palliative medicine physician for a leadership position in a program in Massachusetts
OUTLINE OF ESSENTIAL JOB RESPONSIBILITIES|FUNCTIONS TO INCLUDE BUT NOT LIMITED TO:
Direct care and consultation to adult hospitalized patients
Collaboration and engagement with the interdisciplinary team (Nurse Practitioner, nurse, case manager, social worker, physical therapist, etc.)
Care Coordination and discharge planning
Enhance Collaboration with community resources and hospice and help establishing processes for safe transition and care across the continuum.
DUTIES AND RESPONSIBILITIES:
Provides inpatient consultation services in palliative care
Functions as a leader of the interdisciplinary team to establish a written plan of care at intervals specified in the plan
Facilitates clarification of patient and family goals of care
Consults with the attending physician and facilitates access to appropriate supportive care services
Educates staff and coworkers
Provides full and appropriate documentation of patient care services to support professional billing for these services
Participates in on-call coverage with the interdisciplinary team
Participates in interdisciplinary team meetings to review active cases
Assists in the development of standards of care
Adheres to the practice of confidentiality regarding patients, families, staff, and the organization
Provides prompt, high-quality treatment to all hospitalized patients
Maintains knowledge of coding and working with team members to have coding match services rendered and documentation (the hospital will bill for Physicians services and revenue accrues to the hospital)
Instructs, communicates and meets with the hospital staff in the provision of appropriate medical care
