Outstanding opportunity for an experienced Registered Nurse
to serve as our Community Health Worker Program Supervisor. We are looking for
an RN who is experienced and passionate about community health. The RN
Community Health Worker Program Supervisor will be responsible for the
supervision and development of the Community Health Worker (CHW) Program.
You will lead in the integration of CHWs both in clinical operations as
well as community outreach and community efforts to address chronic disease and
infectious disease throughout the service area. The RN CHW Program Supervisor
will foster community relationships and provide guidance and mentorship to
CHWs.
Minimum Qualifications – Education,
Experience, Certificates & Licenses:
- Degree from an accredited RN, BSN, or MSN program.
- Current Unrestricted Arizona State Nursing (RN, BSN, or MSN)
license. - At least three years’ experience working as RN, BSN, or MSN
- Cardiopulmonary Resuscitation (CPR) certification required.
- Fingerprint clearance card required.
- Driver’s License and Proof of Insurance.
Required Language Skills:
- Ability to comprehend and compose instructions,
correspondence, and communications in English in both oral and written
format. - Bilingual in English and Spanish is preferred but not
required.
